1. NSO authenticated birth certificate
2. Original copy of your baptismal certificate with my name and signature included in the list of your ninangs.
3. Baptismal picture showing me carrying you in your baptismal gown.
4. A copy of the baptismal invitation card
5. Marriage certificate of your parents
6. Recently issued NBI, Barangay and Police Clearance and Cedula.
7. Certificate of good moral character from the priest that baptised you.
Gifts distribution would be strictly from 9 am to 10 am only. Please come on time, no extensions or special deliveries allowed.
